Applying for the DTC involves two key parts and one essential form: Form T2201, the Disability Tax Credit Certificate. First, the person with the impairment or their legal representative completes Part A of the form, which includes personal information and the years for which you are applying. Second, a qualified medical practitioner, such as a doctor or nurse practitioner, completes Part B to certify the nature and effects of the impairment. You can submit the completed T2201 form to the CRA either online through your My Account or by mail.
